Help Your Heart

Heart disease is America’s No. 1 killer. It effects both men and women and almost everyone could improve their heart health. The good news: there are a lot of things you can do to boost your heart health that don’t cost a lot of money, and aren’t complicated or difficult.

Eat less fat- Everyone knows this, but few people know how easy it is to make small changes in your diet that have big impact. Be conscious of what you are putting into your mouth. You want chips and dip while watching the game? Try baked tortilla chips and salsa instead. Like to bake? Substitute applesauce for vegetable oil in your cake. Going to a BBQ? Grilled salmon is just as easy and delicious as a hamburger…with less saturated fat. When you make small substitutions to your diet it doesn’t seem as painful, but makes a big difference to your heart!
Eat more fruits and vegetables- The reason for this is two-fold. Not only do you get fiber and nutrients from fruits and veggies, which can help you process your foods efficiently; you also fill your stomach with low-fat, nutrient-dense foods leaving less room for the fatty stuff.
Don’t skip breakfast- FACT: Most heart attacks occur between 7 a.m. and Noon. There is no conclusive data explaining this, but researchers have found that your platelets (the cells that help your blood to clot) are the stickiest during that time. Eating makes the platelets less sticky and possibly less likely to clump together and form a blockage in an artery.
Lower your cholesterol- Depending on your genes, eating right may not be enough to keep your cholesterol at healthy levels. If you find after changing your eating habits your cholesterol is still high, your doctor may prescribe a statin to help lower it. Be consistent in taking your medication and make sure your doctor monitors your levels closely to ensure good heart health.
Keep your blood pressure in check-This too may be genetic. You may have inherited more than your father’s green eyes. Monitor your blood pressure and if your doctor prescribes medication, be sure to take it as directed.
Find your inner peace- Do you blow up when you are stressed? Over-react when you are frustrated? Studies have shown that people who over-react to stress are more likely to have reduced blood flow to the heart. Relaxation techniques aren’t just for your flaky friend Sunshine anymore…yoga, meditation, deep breathing, stretching and massage are great ways to relax your body and increase blood flow to your heart.
Invest in anti-oxidants-Vitamins A, C and E, beta-carotene, and CoQ10 are important to your heart. These will slow plaque formation in your arteries, keeping the pathways open for your blood to carry oxygen and nutrients to your cells.
Stop Smoking- Smoking isn’t just bad for your lungs, it also constricts your blood vessels and increases your risk for plaque formation, both of which make it hard for your blood to get around. The good news is that a few years after quitting, your risk of heart attack goes down dramatically.
Move it!- A sedentary lifestyle is one of the greatest risk factors for heart attack death. You don’t need to be a triathlete to reap the benefits…a half hour of moderate exercise (like brisk walking) can make all the difference. You say you don’t even have time for that? Incorporate exercise into your everyday life. Take the stairs at work, park on the outer edge of the parking lot and hike in to the grocery store, hide the remote for the TV, join your kids in a basketball game, there are plenty of little changes you can make to add activity and fun to your life!

Fibrocystic Breast Disease

Many women have been told they have Fibrocystic Breast Disease, but few of us understand what that means. Fibrocystic Breast Disease (usually abbreviated FCB) is the most common disorder of the breast. Most cases are in women 30-50 years old and are thought to be caused in part by estrogen. That’s why FCB is not as common in post-menopausal women-they have lower levels of estrogen. FCB may produce a lump in the breast that is discovered by accident, but often there is pain or tenderness in the breast that sends women to the doctor. Usually, a woman with FCB will have multiple lumps in both breasts that rapidly fluctuate in size with her menstrual cycle. Because the lumps caused by FCB are frequently indistinguishable from breast cancer on exam, suspicious lumps should be biopsied. Usually this can be accomplished with a small needle inserted into the cyst to remove material for testing. A breast ultrasound is also helpful in differentiating a solid cancerous mass from a cyst like those caused by FCB. Once a diagnosis of FCB has been made, there are several easy ways to reduce the breast pain until the cysts shrink.

Avoid trauma to the breasts and wear a supportive bra night and day.
Decrease consumption of caffeine as caffeine is thought to stimulate cyst development. This means coffee, tea, sodas, chocolate and many headache medications. Read labels!
Some women have reported relief of symptoms with vitamin E and/or Evening Primrose oil.
Discuss hormone treatments with your doctor. You may benefit from small doses of progesterone to keep your estrogen levels in check. Periodic monitoring of blood levels is necessary to ensure you still have enough of your own estrogen to protect your heart and bones while decreasing your breast pain.

Fibrocystic Breast Disease is a benign condition that usually doesn’t cause complications in your everyday life. However, if you have been diagnosed with FCB, you have a slightly higher risk of developing breast cancer. This is because many women with FCB are so used to finding lumps they are no longer concerned about them, and often will ignore the lump instead of having it checked out. Not all lumps are benign. Be sure to examine your breasts monthly and have a clinical breast exam and mammogram yearly. Symptoms should subside as you enter menopause and regular monitoring will make sure any more serious problems are caught early enough to treat.

Protect Your Bones

Many of us don’t realize that calcium is vital to our entire body, not just our bones. It is the most common mineral in the body and is required for proper function of our brain, our heart, and many other organs. Priority is given to the body’s organs. Not eating enough calcium-rich foods can cause the body to pull the needed calcium out of our bones, causing them to become weak and sponge-like. This condition is called Osteoporosis and can cause bones to fracture during every day activities.

The easiest and least expensive ways to protect your bones are through your food and lifestyle choices. What you put in your mouth will affect your total health in the long run! Some of basic suggestions for strong, healthy bones are as follows:

Make sure you get 1200-1500mg of calcium per day, divided into 3 doses
Vitamin D is necessary for your body to absorb the calcium-drink fortified milk, take a supplement, or get some sun!
Maintain a moderate intake of phosphorus-too little and your body can’t form bones and teeth-too much and it will upset the calcium balance in your body. Carbonated beverages and too much protein in your diet will give you too much phosphorus
Get enough fiber for proper function of the digestive tract, but not so much that it decreases the absorption of calcium. Up to 35 gm per day should be enough
Lighten up on caffeine! Not only does it cause sleep issues, but more than 3 cups of caffeinated beverages per day can cause a loss of calcium in your body
Alcohol has a toxic effect on the formation of bone cells, save your drinks for special occasions, not every day
Cigarette smoking has been shown to increase bone loss and decrease estrogen levels (estrogen protects your bones). Quitting smoking is not only good for your lungs!
Regular weight-bearing exercise helps prevent bone loss. Try adding a few extra steps everywhere you go by parking further away from your destination, using the stairs instead of the elevator, etc. Every little bit helps!
Make sure your estrogen levels in your blood are high enough to protect your bones-a decrease in estrogen with menopause corresponds directly to an increase in bone loss. Menopause is a time when women are especially vulnerable to bone loss.
Check out your diet for simple ways to increase calcium intake-dairy products, dark green leafy vegetables, tofu, fish and whole grains are good sources of calcium. Some juices, grains and dairy products are fortified with calcium as well.

If you and your physician have decided you should take a calcium supplement, how do you know which one is best? Generally Calcium Carbonate is recommended because it has the highest amount of absorbable calcium. Less effective are Calcium Citrate, Calcium Lactate and Calcium Gluconate. Bone meal and dolomite are not recommended because they may contain toxic substances such as lead, arsenic and mercury. Chelated Calcium is expensive and generally no better than any other form of calcium. Make sure you take no more than 2000mg of calcium per day, as long term use of large doses can cause unhealthy side effects. Your physician can help you determine the correct dose for you as an individual.

Digital Mammography

The usual recommendations are that women over 40 years old get a mammogram every year. While this is a valuable screening tool, it’s not at all comfortable and the image obtained can be hard to read. Advances in mammography technology are changing all that. Many sites are now offering digital mammography. The images are much clearer, often making additional views unnecessary. Along with the digital mammography service, many sites are trying to make the process more comfortable as well. Breast pads are now being offered so you feel less compression and no cold metal plates! The facilities themselves are undergoing change, becoming more like a day spa with artistic interiors and little “goodie bags” being given to patients. One local company is leading the way towards a more comfortable, accurate mammogram experience. The Imaging Center of Idaho offers a friendly and professional staff, the latest technology, a comfortable interior and soft padding on their mammography plates. I had my screening mammogram there recently and did not feel any embarrassment or discomfort at all. I came away with a clean bill of health, my dignity intact and a “goodie bag” filled with breast health information, mints and a pair of pink gardening gloves. It’s time that medical professionals realize that it’s not enough to follow the latest studies and buy the latest equipment. I applaud the movement towards personalizing treatment-Do I feel cared about? Is the staff knowledgeable and concerned with my emotional well-being as well as my physical care? Can the facility get accurate test results the first time and explain them to me? Even though mammograms are not my favorite way to pass the time, do I feel comfortable going back next year? After my experience with digital mammography and with the Imaging Center of Idaho this year, I can answer “yes” to all these questions. I encourage all women to consider digital mammography for their yearly screenings and ask your doctor who they recommend. Facilities are upgrading all over the country and your mammogram doesn’t have to be a horrible experience anymore.

Assessing Cardiovascular Risk

Traditionally, your doctor would run through several risk factors to determine if you were at risk for cardiovascular disease:

Are you a smoker?
Do you have high blood pressure?
Do you have high cholesterol?
Are you overweight?
Do you have Diabetes Mellitus?
Are you sedentary?
Do you drink excessive amounts of alcohol?
How do you deal with stress?
Are you male?
Do you have a family history of cardiovascular disease?
Are you over 65?
Are you Hispanic, American Indian, or Native Hawaiian?

All of these factors could put you at higher risk for cardiovascular disease. If you have 2 or more risk factors, there is a new test your doctor should consider. The PLAC test is a blood test that measures the level of Lp-Pla2 present in your system. This enzyme is produced by plaque in the body and indicates inflammation and the formation of rupture-prone plaque. It is the only blood test approved by the FDA to aid in assessing risk of coronary heart disease and ischemic stroke from atherosclerosis. If your Lp-Pla2 levels are elevated, your doctor may want to pursue more aggressive therapy to lower your risk. If your levels come back normal, published data suggests you are at lower risk and can take a more conservative approach.

The PLAQ test is a simple, yet thorough test; you don’t have to be fasting, you can continue to take medications uninterrupted, and the results are not falsely elevated from infections, obesity or rheumatologic disorders like the CRP (C-Reactive Protein, another common screening test for determining inflammation).

Half of all heart attacks occur in people with low to moderate cholesterol levels. One third of all strokes occur in people 45-65 years of age with all levels of cholesterol. Cholesterol levels are not a reliable predictor of heart attack or stroke- the PLAQ test is. Often the PLAQ test is not covered by insurance as it is considered preventative. But, at under $100 it may be worth paying out of pocket for if you have risk factors. Ask your doctor for more information on the PLAQ test.

Vitamin D and Breast Cancer Risk

Several studies have recently shown that women that are deficient in Vitamin D are more likely to develop breast cancer. The lower the level of Vitamin D in their blood, the higher the grade of cancer they develop and the higher the risk of mortality. In these studies, the association between Vitamin D levels and disease free survival were not related to age, BMI (body mass index), insulin level, and tumor stage/grade, estrogen-receptor status, chemotherapy or tamoxifen use. This implies that adequate levels of Vitamin D in your blood could protect you from breast cancer. If you do end up with breast cancer, maintaining adequate Vitamin D levels along with your treatment may prevent death and increase your chance of disease free survival.

Vitamin D deficiency can also harm your bones. Your body requires Vitamin D to absorb Calcium and make it available for use. If you don’t have enough Vitamin D, Calcium is not absorbed and your bones become weak, leading to rickets in children and osteoporosis in adults.

If you and your doctor decide that you would benefit from supplementation, your serum 25 hydroxyvitamin D levels must be measured before and during treatment. High levels in your blood could lead to the permanent deposit of minerals in the heart, lungs, and kidneys. Symptoms of toxicity are nausea, vomiting, pain in joints, and lack of interest in eating food. Continuing at toxic levels can result in death.

If you have any questions about your Vitamin D levels, talk to your doctor. Open communication with your doctor is one of the most important tools you have in the pursuit of good health.

Why Eliminate Compounded Medications?

By using a compounding pharmacy, your doctor can have your medication custom-made for you. A compounding pharmacist can eliminate allergens specific to you, put hormones into a cream form to make them safer, and make your medication fit your lifestyle. Do you travel a lot? Maybe you would prefer your progesterone to be in a capsule. Are you prone to blood clots? An estrogen cream would be a much safer alternative for you. Allergic to peanuts? A compounding pharmacy can create a progesterone capsule that is a safe alternative to Prometrium for you. There are many great reasons to use a compounding pharmacy-why is the FDA cracking down on them?

First, a little history lesson. In 1942, a drug called Premarin was first marketed for menopausal symptoms. It received FDA certification in 1972. When Wyeth’s (the company that manufactures Premarin) patent expired, several smaller companies created generic, cheaper versions. In 1990’s, Wyeth succeeded in having the generic versions of Premarin taken off the market. Wyeth proposed that the generic versions were unsafe because they were faster-acting. Incidently, Wyeth was selling it’s own fast-acting version of Premarin in Canada at the time.

In 2002 and 2004, findings were released from the Women’s Health Initiative study that showed risk of venous thrombosis was doubled with estrogen/progestin combinations (PremPro, a sister drug of Premarin), and an increased risk of ovarian cancer. Women everywhere panicked and went off their hormones. Wyeth lost $1 billion in net revenue from Premarin and PremPro the very next year. Women started feeling menopausal symptoms again after stopping their medications and started searching for safer alternatives. Many women and physicians turned to bio-identical hormones. Citing studies that showed estrogen is safer applied to the skin, rather than in a pill, physicians started prescribing compounded hormone creams. Micronized progesterone was found to be much safer than the synthetic progestin found in PremPro and could be compounded into a cream or capsule.

In 2005, Wyeth petitioned the FDA to “initiate enforcement actions” against the compounding pharmacies. After that was denied, Wyeth went public and told consumers that the compounding pharmacies were unsafe. They initiated an educational campaign that told half the story to frightened consumers. The FDA then reversed their decision and compromised on a limited crack-down on compounding pharmacies, concentrating on the dispensing of estriol.

Estriol is the most gentle estrogen produced by the body. It is the main estrogen released by the placenta during pregnancy and is thought by many experts to be protective against high levels of estradiol. It is a US Pharmacopeia ingredient and is perfectly legal to compound. Compounding pharmacies are heavily regulated by state boards of pharmacy already, the FDA exists to regulate mass manufactured products. Estriol is often mixed with estradiol to make a compounded cream that is the top competitor of Premarin. The FDA has stated that estriol is not FDA approved and that it has not been proven to be safe. They do note, however, that “there was no reason to believe that the risks of compounded estrogens and progestins would be any different that the risk reported for FDA-approved hormone replacement drugs.” (Dr. Kathleen Uhl, FDA’s Office of Women’s Health)

Still wondering why the FDA is going after compounding pharmacies? Steve Silverman of the FDA’s Office of Compliance stated that this latest action (in 2008) was “solely in response to a 2005 citizen’s petition from Wyeth Pharmaceuticals, which asked the FDA to crack down on the hormone replacement products marketed by compounding pharmacies.” The same Wyeth Pharmaceuticals that lost over $1 billion/year to compounding pharmacies.

Testosterone and the Prostate

It’s long been thought that higher levels of testosterone in men would lead to an increased risk of prostate cancer. This belief has led many men to accept the side effects of testosterone deficiency as an inevitable part of aging: weight gain, loss of muscle tone, lack of energy, and a decrease in sexual enjoyment. However, recent studies have shown that testosterone therapy is not associated with increases in prostate cancers or any other prostate illnesses.

Many studies have shown that severely limiting testosterone can cause prostate cancer to shrink temporarily. From that, physicians have concluded that raising testosterone levels would make prostate cancer grow. However, no evidence has been found that links testosterone treatment with prostate cancer. In fact, prostate cancer becomes most prevalent at the time in a man’s life when natural testosterone levels decline. In 2002, a study published in the International Journal of Andrology found that men who received testosterone therapy had decreased prostate size, lower PSA numbers and an improvement in urinary symptoms.

Per Dr. Eugene Shippen, author of The Testosterone Syndrome, “All organs of the male reproductive system, including the prostate, tend to stay healthy in the presence of adequate levels of key hormones, including testosterone. Normal concentrations of testosterone and its more powerful derivative may well be harbingers of prostatic health, not illness.”

Make sure you are doing your part to maintain good prostate health- have a digital rectal exam every year, have a blood test called the PSA yearly, and ask your doctor about testosterone therapy to alleviate symptoms of deficiency.

Vaginal Discharge and Itching: When should I see a doctor?

A small amount of vaginal discharge is completely normal. Glands inside your vagina and cervix make small amounts of fluid that constantly carry old cells out of the vagina. When you ovulate, breastfeed, or are sexually excited, the discharge can get thicker, but the color remains clear or milky and it does not smell bad. There are several conditions that can cause an odor, a change in discharge, and/or itching. See your doctor if you experience any of the following:

a sudden increase in the amount of discharge
a change in color or smell of discharge
irritation, itchiness or burning in or around your vagina
bloody discharge when you aren’t having your period

What could be causing your symptoms? There could be many causes, but let’s look at some of the more common problems:

Yeast Infection- an overgrowth of yeast fungus in the vagina. Usually not sexually transmitted, you’re more likely to get a yeast infection from antibiotic use, pregnancy, hormone imbalance, diabetes, or being hot and sweaty for long periods. You may have a “cottage cheese” looking discharge, itching, burning, and pain during intercourse. Treated with a vaginal creme that is sold over the counter. Most women have the best results with the 7-day treatment. If that doesn’t work, your doctor can prescribe a pill that will get rid of a yeast infection in one to two doses.
Bacterial Vaginosis-a bacterial infection treated with antibiotics. Not usually sexually transmitted. You may have a clear or colored discharge that may have a fishy smell, especially after intercourse. Must be treated or it may cause more serious infections in the fallopian tubes or uterus.
Trichomoniasis-an infection caused by a protozoan parasite. Usually caught by having unprotected sex with an infected person. Treated with one dose of antibiotics. Signs of trichomoniasis in women include a frothy, yellow-green discharge, strong odor, pain with intercourse, and genital itching.
Chlamydia-a sexually transmitted bacterial infection. 3/4 of women with chlamydia will have no symptoms. Some may have abnormal discharge and burning with urination. Treatment usually consists of a single dose of an antibiotic.
Gonorrhea-a sexually transmitted bacterial infection. Most women infected with gonorrhea have no symptoms. If you do have symptoms, they can range from burning sensation while urinating, to increased vaginal discharge and bleeding between periods. Treated with antibiotics.

These are the most common causes of vaginal discharge and itching. If you experience problems, your best course of action is to see your doctor to be properly diagnosed and treated. If you are one of the women who gets frequent yeast infections however, and know the symptoms- do try the over the counter remedies first. If that doesn’t work, follow up with your doctor.

Hormones and Breast Cancer

A German study has been published that shows previous hormone therapy use appears to be a factor increasing survival in women with breast cancer. It’s speculated that hormone therapy prevents bone metastases (the spread of disease from one organ to another). The 1072 breast cancer patients who were studied were divided into 3 groups; premenopausal, post-menopausal with previous hormone therapy use, and post-menopausal with no hormone therapy use. The authors of the study found that previous hormone therapy users had significantly higher rates of survival after breast cancer than nonusers-the 5 year survival rate for previous HRT patients was 92.8% versus 82.2% for non-HRT users.

It’s important to keep up-to-date on the latest research-your health may depend on it. Ask your physician what the latest findings mean to you.
